Medical Records Technicians are the individuals responsible for assembling and maintaining the medical records of patients who receive health care. These records, which are vitally important in treating the patient, contain a compilation of the following information:

  • Medical history
  • Observations
  • Surgical interventions
  • Treatment outcomes
  • Descriptions of symptoms
  • Results of examinations
  • Reports of x-rays and MRIs
  • Results of laboratory tests
  • Diagnoses
  • Treatment plans

Technicians verify the completeness of a patient's initial medical chart and make sure that all forms are properly filled out and authenticated. They regularly consult with physicians and other health care professionals for clarification of patient diagnoses and to obtain any additional information required for a patient's records. Technicians also tabulate data for future analysis, provide documentation when needed for use in legal actions, and compile information used to classify reimbursement status of medical procedures by insurance companies.

Responsibilities

Medical records technicians organize and maintain patient records in a manner consistent with the requirements of all components of the health care system: medical, administrative, regulatory, legal, and ethical. A partial list of their duties includes the following:

  • Process patient admission and discharge documents
  • Review records for accuracy, completeness, and compliance with regulations
  • Enter data into computers regarding demographic characteristics, disease history, diagnoses, and treatments
  • Release information to persons and agencies according to regulations
  • Protect the security of medical records to ensure that confidentiality is maintained
  • Develop health record indexes and storage and retrieval systems to collate information

The specific day-to-day duties of a medical records technician will vary with the nature of the facility for which he/she works. In small facilities, a credentialed medical records technician usually assumes a variety of technician duties which often includes management of a department. In larger or medium-sized facilities, there are usually several technicians and very often, each will specialize in one aspect of health information. In these types of settings, management duties are usually assumed by an administrator. Examples of specialists in this field include the following:

  • Medical Record Coders specialize in coding patients' medical information for insurance purposes. Based on their knowledge of disease processes, these technicians assign a code to each diagnosis and procedure. They then utilize specialized computer software to classify the patient as belonging to one of several hundred "diagnosis-related groups" (DRGs). The DRG classification determines the amount of reimbursement the hospital will recover from the patient's medical insurance carrier or from Medicare to cover the cost of the procedure. Coders are also responsible for using a variety of coding systems other than DRG to enter classifications for other applications such as doctor visits, ambulatory settings, or long-term care.
  • Cancer Registrars specialize in the registry of cancer patients. Based on reviews of a patient's records and pathology reports, they assign codes for the diagnosis and treatment of cancers and also certain benign tumors. Registrars will enter information into regional or national databases and then follow up by conducting annual checks on patients in the registry to track and/or update their treatment, survival, and recovery. Information in the registry is used by physicians and public health organizations for a number of reasons including the derivation of survivor and success rates of various types of treatment, identification of geographic areas having high incidences of certain cancers, and the targeting of potential participants for clinical drug trials.

Job Characteristics

Most medical records technicians work a standard 40-hour week, although some overtime may be required. Those who are employed in hospital settings are often required to work evening or night shifts and/or work on weekends, due to the fact that hospital health information departments are usually open around the clock seven days a week.

Technicians typically do their work in pleasant and comfortable office settings. Unlike their counterparts in most health-related occupations, technicians usually have little or no direct contact with patients. There is some degree of stress in the job due to the ongoing demands for accuracy and the need to pay close attention to detail. Also, technicians are susceptible to the types of discomforts commonly experienced by people who work at computer monitors for prolonged periods of time; notably muscular pain and eyestrain.

To be successful in this job, individuals need good communication skills in order to effectively interface with insurance companies and other establishments. Technicians also need to be detail-oriented and extremely diligent, as accuracy is essential to the job. To an increasing extent, proficiency with computer use is becoming an extremely valuable asset for this profession.

    When tattoo artist Brandon Bond heard about a new tattoo ink that could be removed with a single laser treatment, he wasn't just skeptical — he was ticked off. The Atlanta-based designer considers the work he has inked on everyone from rapper 50 Cent to champion boxer Floyd Mayweather, Jr. to be pieces of art, almost sacred, so marketing them as disposable was nothing short of an insult.

    But even as Bond scoffed at the ironically named Infinitink, which just became available this spring, he could relate to Americans' love-hate relationship with their body art. Cherished as symbols of independence and individuality, tattoos now adorn the flesh of a quarter of all adults under 50. Yet recent studies have also found that about a sixth of everyone who gets one winds up regretting it. Case in point: Bond sports two full sleeves' worth of intricate patterns, but admits there are a few designs on his arms that he'd like to erase altogether. "A lot of the stuff I had gotten as a youngster was very subpar," he says. 

    With such conflicting sentiments in mind, Harvard dermatologist and professor Rox Anderson developed the biodegradable Infinitink by encapsulating the pigment in tiny plastic beads that dissolve more easily than regular ink when struck by a laser beam. But there was a problem: tattoo artists hated to use it because it was too thin (which made it look washed out) and the micropolymer beads were incredibly expensive, says Bond, who now works as a consultant for Nuvilex, which makes Infinitink.

    So the company decided to reformulate its ink and hired a chemical engineer from Dupont, who replaced the expensive beads with a technology, already used in ink jet printers to conserve ink, that helps smaller quantities of pigment create the same vibrant results as regular inks. Ultimately, come regret time, that means there's less ink that needs to be removed, which means less time spent getting zapped by a laser. With this revised formula, Infinitink tattoos still cost as much as 50% more than regular designs, but their removal is a bargain since it requires many fewer sessions — which typically cost $200 to $500 a pop — to shatter the ink into small pieces that get absorbed into the lymph nodes.

    October is finally here and all seems right in the world. Well, except for that whole possible terrorist threat throughout Europe, but let's toss that one aside to the media hype. Suddenly, there's an insatiable craving for anything pumpkin flavored, the brutal wait for an outbound train is even more hellish and it's finally acceptable to admit that you're wearing long-johns underneath your jeans … well, maybe. Life - if only for a mere 31 days - slows down. The hassle of back-to-school assimilation is over and there's a solid amount of time before the holiday madness begins. But there's a distinct somberness breezing through the autumn air.

     

    This time last year, we lost three students in the span of a month and a half. In fear of another bereavement notice, opening our university e-mail became a cringe-inducing challenge and the palpable sadness throughout campus couldn't be ignored. Campus was eerie for a while. Whether we knew the girls personally or not, students and faculty appeared to be in a trance, simply going through the motions of attempting to conduct a "normal" day.

    Now, a year later, the same solemn feelings seem to have resurfaced. While remembering the students we've lost, it's difficult to stifle the sadness.

    Five years ago, I began compiling my most beloved possessions and stashing them away in the little pocket of my bag.  Each held its own sentimental meaning; photographs of my parents, notes from family members and friends and a menagerie of miscellaneous items that I couldn't seem to part with. What began as a minor compulsion soon manifested into toting around a hoarded, material version of the past. Physically carrying around sentimental parts of my past brought a sense of security and the comfort of knowing that remnants of my memories were at my side was oddly comforting.

    After moving to Chicago, I toyed with the idea of nixing carrying around the bulk of my most cherished possessions. That thought was merely fleeting. I blame stubbornness. And when my bag was stolen this summer, there was no one to blame but myself. On the plus side, I can now think of myself as a true Chicagoan.Or at least that's what I heard. Whether out of truth or an attempt to placate me, I'm unsure.

    This 12 week evening course teaches students basic design and decorating principles. It is for those who wish to work on their own as well as for people already working in the interior design industry. This intensive program of lectures, combined with practical design work and factory visits is constructed to provide a comprehensive introduction to the field of interior decorating. Students are trained to have a knowledge and understanding of design elements, concepts and methods, communication (both visual and verbal) and understanding of materials and their use; interior movements and styles; and business studies, including teamwork and leadership.

    There will be visits to main fabric showrooms to show students what is available locally. Students are encouraged to develop their own individual design ideas in the production of their main project - the decoration of a small penthouse apartment. The project requires the communication of proposals through plans, sections and-or elevations, 1- and 2-point perspective views, concept and sample boards, and compilation of quotation and contract documents.

    On completion of the course students will be competent enough to tackle small projects on their own with confidence and will possess functional understanding of the elements and principles of interior decorating. Ten trades may make you money, but one trade could ruin you in no time. Find your threshold and only increase it if you are profitable.

    Will I make money or lose money trading?

    This question is hard to answer. The odds are against the average retail trader and you will most likely lose money. Some people only lose a little bit but a lot of people get so emotionally involved that they end up blowing their accounts completely. This is something you need to avoid at all costs.     Freight shipping volume has been picking up in 2010 and while that is great news for the world economy it has caused an unexpected shortage of containers. According to the July cover article in American Shipper, this shortage in shipping containers is being sited as one of the main causes for the rise in freight rates and has led to difficulty in moving product to buyers. Many shippers have complained about signing contacts with shipping lines only to have rates rise the following week or to be denied space completely. The average price for a 20ft shipping container has grown from about $2,000 to $2,700.

    One of the major reasons for the container shortage was the drop in production of new shipping containers in 2008 and 2009.  From 2004 through 2008, TEU supply grew by an average of 8% per year. Once the world recession hit, trade dropped dramatically and so did demand for shipping containers. There was a 95% reduction in shipping container production in 2008 and almost no containers were built in 2009. In response to the drop in demand many of the factories that build the containers, mostly based in China, were forced to shut down. Now with the increased demand for there is a reduced capacity for factories to produce new containers. Some estimate that it will be until 2011 before production will be able to catch up with demand.

    While this shortage is bad for manufactures and ultimately consumers, container-leasing companies are appearing to benefit in the short run from this problem. Stocks of publicly traded container-leasing companies have reached 52-week highs this spring and summer. Additionally many leasing companies are taking the opportunity to increase their share of containers. Historically, liner carriers have owned about 55% of the world shipping containers with leasing companies owning the remained 45%.

    Medical billing is a growing industry in North Jersey, fed primarily by unemployment, the appeal of a recession-proof business and demand for a job that has become increasingly frustrating.

    Despite some highs and recession-based lows, the surviving start-ups are growing enough to put tiny dents in the state's high unemployment rate by hiring staff to handle new doctor offices and hospitals.

    "We've definitely seen an increase in people looking to get certified in medical billing," said Larry Weston, president of the American Medical Billing Association, in Davis, Okla. The association offers certification through medical billing schools, and has about 100 members in New Jersey, he said. The American Academy of Professional Coders in Salt Lake City has 99,014 members.

    The requests for certification rose once the economy took a dive, said Weston, because health care continues to grow, he said.

    Organizations and billing schools talk up seemingly simple start-up requirements of a home-based business that needs only a course, computer and software. But billers describe the work as frustrating and highly competitive, needing great attention to detail to submit patient claims accurately or risk payment to doctors and themselves.

    Theresa Wilson formed Practice Synergy LLC in Butler in May after being laid off twice in three years, she said. She spent $2,000 for a four-month medical billing course to become certified, and classes in running a business.

    Wilson also joined New Jersey Association of Women Business Owners and mentored with a chapter president and veteran medical biller, Debra Courtright, who helped her get her first customer — a doctor's office in West New York.

    "The big misconception is that it [the business] is easy," said Wilson. "It's not easy."

    Wilson said her background in collections as a compliance officer and as an underwriter gave her relevant skills in following up on unpaid bills and in tracking medical regulations.

    "You have to pay attention to detail as an underwriter," said Wilson. "When you're a medical biller, you have to pay attention to detail. And being a compliance officer in the finance industry helps with the compliance end of medical billing."

    Wilson said the growth of the business enabled her to move out of her home office, rent space, and hire a part-timer — and soon a second. She said medical code changes coming down the pike will also help boost sales.

    Doctors say they outsource billing because the reimbursement process can be overwhelming. It requires repeatedly submitting claims, checking their status, making sure the coding is correct and tracking constant policy changes.
